Jerry Salkowe, M.D., HANYS' Vice President, Clinical Affairs, has been driving the PCMH change movement and will share proven, effective strategies to bring even your most change-resistant provider along the change continuum. Additionally, we will spotlight a physician change agent who is in the heart of driving change now within a system and/or site.

• outcomes and data to support the PCMH delivery model.
